arm-usb-tiny has died?

I believe my arm-usb-tiny has died was hoping someone might help me confirm. I’ve been using it successfully for a few months when the following occurred:

  1. the led on the arm-usb-tiny does not light

  2. I verified the usb cable works via another device

  3. I get the following when trying to run openocd

Anyone have this device die on them?

Well I purchased another debugger and it still doesn’t work. This leads me to believe its the board (Olimex LPC-H2294). Anyone know how I can further debug this issue?
