Help with power supply pinout

Hi, I have a dc-dc high voltage power supply that I bought about 5 years ago. It has no identifying marks on it, but I think it came from SparkFun.

I’m looking for the pinout / datasheet for this.

Does anyone recognize it?

Do you have a bottom picture? It doesn’t appear to be anything we’ve made

Here’s the bottom. Maybe it’s just a generic ebay thing, I don’t remember.
