I tried to designing a simple breakout board for connecting an LCD from Nokia 6060/6010 to SPI of panda II.

I would add some bypass capacitors (at least a 100nF between Vcc and GND near the connector). I would also use wider traces for VCC, GND, and the LED ones.
Do you need any mounting holes?

Yes, I'll make all traces at least 0.7mm ( currently pads are 0.25mm ).

It's just that way in the image. On the pcb it'll be nice and tidy. :)
