This depends on the CAD package you are using. Could you inform us on the CAD package you are using?

If you’re talking about how to define the holes in the files you send to the PCB manufacturer, I don’t think there’s any standard way to do this.
The best thing to do would be to talk with them about their requirements. In the past, I’ve defined rectangular holes using one of the Mechanical layers (in Protel). I then generate & send the Gerbers to the manufacturer with clear instructions regarding which Gerber file is for what.
Most PCB manufacturers will take a Gerber file for line slots to cut in a board - simply use your CAD tool to draw on a new layer (normally the “Mill” layer). Talk to whoever builds your boards - this is another operation which must be performed and may raise your cost.
